CANCELLATION POLICY
We strive to deliver the best possible service to our patients. Please acknowledge and agree to the following:
1. If you wish to cancel your appointment, we require a minimum of 48-hours notice. If you cancel your appointment <48 hours of your designated appt time OR you do not show up to your appointment, you are subject to a $50 late/reschedule-fee.
2. If you wish to be seen on the confirmed appointment date, we require patients to completely fill out and send back the medical forms 24 hours prior to the confirmed appointment date. If you do not fill out and return the medical forms and consents a minimum of 24 hours before your appointment, you may be asked to reschedule.
